You now have all necessary to request a baptism.

- If this is the first page you are visiting rather than the general page in which we list other information of relevance, we want to make sure that you read this before proceeding to filling out a request / application form to baptize your child.
  
  This only applies to Infant & Young Child Baptism (Under Age 7)
  
  Attend Formation
  Parents and godparents must attend formation to help guide the child in the faith.
  Submit Baptism Request Form
  To submit the application, you must include all required documents when applying.
  
  Eligibility
  This parish community serves families who live within parish boundaries and/or are registered parishioners. Parishes are stable local communities led by a pastor. Registration reflects an intentional commitment to the parish (receiving a sacrament or service alone does not automatically constitute registration).
  
  Please check the Boundary Map to confirm eligibility. If you meet the requirements and have all documents, you may proceed with the Baptism Request Form.
  
  If you are not registered or do not reside within parish boundaries, you are still welcome to baptize at St. Angela with a letter of permission from your pastor.
  
  If you need more clarification, you may schedule a meeting with the director of adult faith formation.

  All the required documents listed below, since you will need to upload them.
  If you believe you fulfill all the requirements but didn't take the formation with us, please schedule a meeting with the director of adult faith formation since there are a few other steps that you may not be aware of.
  Required Documents before you start filling out the form.
  The State Child’s birth certificate (NOT the vital fact form that you obtained at the hospital).
  Godparent(s) form – One per godparent, fully completed and signed by the godparent.
  Proof of baptism preparation/class attendance – For parents
  Proof of baptism preparation/class attendance – For godparents
  
  If Applicable
  Letter of permission to baptize – Required if you are not a registered parishioner or live within the parish boundaries; if you are not sure about this, contact the parish office to verify your registration status.
  Legal guardianship documents – If applicable
  Christian Witness Form - At least one godparent is required before requesting a Christian Witness.
