We recommend beginning the baptism process at least six (6) months before your desired celebration date.To ensure an efficient baptismal process, all required information and documents must be submitted together with the Baptism Application Form.
When completing it, you will upload all required documents that you would have already completed before requesting a baptism date.
A baptism date cannot be secured until all documents are received.

Baptismal Application Requirements
When completing the application, you will provide basic information, confirm your parish registration, and must have the following documents at hand to upload when prompted:
Required Documents- Child’s birth certificate
- Godparent(s) form – One per godparent, fully completed and signed
- Proof of baptism preparation/class attendance – For parents and godparents (at least one parent’s proof is required)
- Letter of permission to baptize – Required if you are not a registered parishioner; contact the parish office to verify your registration status
- Legal guardianship documents – If applicable
The Godparent Form must be fully completed and signed by the godparent. Please ensure the parish of registration and attendance is clearly filled out before submitting the form. This required question is often left blank and must be completed for the process to move forward.
Christian Witness Form - At least one godparent is required before requesting a Christian Witness. Do not submit this form if you have two godparents listed.
