Questions You May Have Before Taking the Survey
What Is the Disciple Maker Index (DMI)?
The DMI is an anonymous parishioner survey designed to provide insight into the level of missionary discipleship within the parish. It also gathers feedback about the Mass experience, preaching, and other aspects of parish life.
Is the Survey Confidential?
Yes. Confidentiality is taken very seriously. Parishioners are encouraged to provide honest and open feedback.
Individual parishioner responses will remain confidential. No one in the Archdiocese or on the parish staff—including the pastor—will have access to individual responses.
Who Will See Our Parish Results?
The parish’s overall results will be shared with the pastor and with the key parish leaders he chooses to involve in reviewing the data.
How Long Does the Survey Take?
The survey provider estimates that the DMI takes approximately 10–15 minutes to complete. For some parishioners, it may take less time.
Who Should Complete the Survey?
Catholic Leadership Institute recommends that individuals 18 years of age and older complete the survey. However, students in 11th and 12th grade may also participate.
What Languages Is the Survey Available In?
The DMI is available in multiple languages to make participation accessible to as many parishioners as possible: English, Spanish, Haitian Creole, Portuguese (Brazilian and European), Polish, French, Canadian French, Chinese (Simplified and Traditional), Tagalog, Italian, Vietnamese, Burmese, Kirundi, Swahili, Kinyarwanda, Korean, and Tongan.
The DMI Survey will be open for responses from September 27 through October 25, 2026.
Prefer a Paper Survey? Available starting October 3/4Paper copies will be available at the Hospitality Ministry Desk in the Church Narthex and in the Parish Office.
Simply take a copy home, complete the survey, and return it to the designated box labeled “Drop Off Surveys Here.”
